All the holes in the swiss cheese lined up and even more holes were man-made! Too bad..
Instrument failure may be poor reporting. Based upon the NTSB preliminary report, the pilot may have failed to engage the generator after starting the engines. How a pilot with so much flight time and training could have screwed up so badly that seven people died is beyond belief. But that happens often.
This is 100% human error. It was also 100% preventable.
